DAY SERVICES
What is Day Services?
Day Services is a program that:
- Enables people with disabilities to be active in their communities.
- Connects persons' individual abilities to their communities.
- Develops and supports successful community participation by persons with disabilities.
Am I eligible for CIC Day Services?
Yes, if you:
- Are at least 18 years of age.
- Have a disability which prevents you or keeps you from connecting your individual abilities to the community on your own.
- Have financial sponsorship or the means to pay for your own services.
Who pays for my CIC Day Services, and how do I enroll?
If you are sponsored by one of the agencies listed below they will pay for the cost of using CIC's day services. They will contact CIC for you to
get services started.
- Community Care of Central Wisconsin (CCCW also known as Family Care).
- Portage County Health and Human Services.
- Other sponsors might be another county Health and Human Services Department or Family Care Sponsor if you are a resident of a county other than
Portage County.
If you are paying for your own services or using self directed services such as IRIS (Include, Respect, I Self Direct) that are paid for by the state, you will contact CIC to arrange for services and payment. Anyone may contact us to get additional information about services and payment.
What may I expect while I am enrolled in CIC Day Services?
- Getting Started:
- CIC staff will get acquainted with you while evaluating your needs.
- You will participate in individual and group activities where you will be assisted in identifying your interests and abilities.
- You and staff will talk about your skills, expectations, goals, and dreams in the areas of socialization, mobility, self-care, communication,
leisure, health/safety, and vocational. With your permission we will talk to your family and friends about these as well.
- The Next Step:
- After three months of assessment you and CIC staff will draw up a plan of ongoing activities.
- These activities will shape your weekly routine.
- This routine might include paid work-if so your other activities will be pursued around work.
- Setting Your Goals:
- Your routines and activities may be as individual as you are.
- They may include such activities as music exploration, fitness routines, shopping, dining out, meeting friends, volunteering, arts and
crafts, hobbies, and employment.
- CIC services will help fill your future with a lifetime of enriching options.
- Your abilities will be connected to the community.

- You will see us out and about in the Portage County community much of the time.
- We also have four "bases" of operation where much of our services originate.
